DAAA National Games Registration Form
Important Information - Please Read Carefully

DAAA will not host National Games in 2017. Instead we support Team USA at the World Dwarf Games. DAAA will host boccia for all ages and ranges of ability at the Denver Sheraton hotel during the 2017 LPA conference on Tuesday, July 4th and co-organizing the East/West Softball game with LPA on Monday, July 3rd. 

Medical Evaluations
Medical evaluations are not required for Boccia.

Registration is Completed ON SITE
All Athletes must complete their registration at the DAAA registration area before competing in any events. On site DAAA registration includes:
  • Athletes under the age of 18 must bring a parent or guardian with them to complete registration.
  • Pick up Participation Number (required to check in and participate at all events)
  • Sign Athlete Code of Conduct and Waivers
  • Sign Family and Friends Code of Conduct and Waivers
  • Pick up final schedule
Boccia participant: Will be for individual and/or team single elimination tournament
DAAA maintains the right to fill incomplete team rosters with independent players at DAAA’s discretion.

    Eligibility
    You are eligible for competition if you are a disproportionate dwarf (diagnosed with chondroplasia) that is less than or equal to five feet in height, or a proportionate dwarf less than or equal to 4 foot 10 inches in height.
    • All Athletes must report to the DAAA Registration Room prior to your first event for check in and sign code of conduct. All Athletes will be assigned a Participation Number that must be presented at check-in for all events

    Waiver

    In consideration of being allowed participating in any way in National athletics/sports programs and related events and activities, I the undersigned: Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own actions, inaction's or negligence but the actions, inaction's or negligence of others, the rules of play, or the condition of the premises or of any equipment used.

    Further, I have fully considered and understand the risks that I could be physically injured as I prepare for, participate in and travel to and from the programs and events of the National Games. I assume these risks.

    Further, I know of no reason why my participation in this program or its events should be exceptionally or unusually hazardous to my health. I assume ALL of the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.
    I agree to release, waive, discharge and indemnify the DWARF ATHLETIC ASSOCIATION OF AMERICA or THE LITTLE PEOPLE OF AMERICA, INC., any affiliates, their respective administrators, directors, agents, coaches and other employees, participants, sponsors and volunteers from any and all liability to themselves, their heirs, and administrators for any injuries, including death, damages, losses or expenses.

    Photo Waiver: Further, I hereby give my permission to the DWARF ATHLETIC ASSOCIATION OF AMERICA and designated agents to take and use my photograph, a video-tape of me or my voice for publication or reproduction in any advertisement, media relations or fund raising efforts of the association, its programs or agents.

    The undersigned have read the above waiver and release and understand that they have given up substantial rights by signing it and sign it voluntarily.
