2018 Orlando DAAA National Games Athlete Registration Form
Important Information - Please Read Carefully


The Dwarf Athletic Association of America (DAAA) will be hosting the 31st National Games during the 2018 Little People of America’s (LPA) National Conference in Orlando, Florida, Saturday July 7 through Wednesday July 11, 2018. Onsite Registration will open Friday, July 6th.
               
DAAA encourages all athletes with dwarfism, no matter the level of ability and skill to join the competition.

Important Registration Information- Please Read Carefully
All athletes who wish to participate are required to register for the DAAA National Games and complete payment.

Medical Evaluations
All athletes are required to submit a current Medical Evaluation. All 2016 Medicals for Boston have expired; therefore all athletes must submit a new form. All WDG Canadian Medical Evals do not apply. Athletes who only participate in Boccia do not need a Medical Evaluation.

Registration is to be completed ON SITE
All Athletes must complete their registration at the DAAA registration area before competing in any events. On site DAAA registration includes:
  • Athletes under 18 must bring a parent or guardian with them to complete registration
  • Pick up Participation Number (required to check in and participate at all events)
  • Turn in current Medical evaluation (Click to download 2018 Medical Evaluation Form)
  • Sign Athlete Code of Conduct and Waivers (Click to download 2018 Code of Conduct and Waiver)
  • Sign Family & Friends Code of Conduct and Waivers
  • Pick up final schedule of events
  • During the games we will post event information and results near onsite registration, on our Facebook page and website.
 
Team Rosters Only Team Captains are allowed to submit team rosters. Team Roster Forms are due June 1st, 2018 8 pm PST. Click here to submit rosters.
Minimum team size for basketball is ten players per team with a maximum team size of twelve players.
Minimum team size for soccer is twelve players per team with a maximum team size of fifteen players per team.
Team captains please check DAAA website for updated rules before the games to share with your team. (i.e. in soccer, there are no headers and no direct kicks). Contact Amy Freeman <athletemanager@daaa.org> with questions.
DAAA maintains the right to fill incomplete team rosters with independent players at DAAA’s discretion.
If you are participating in a team sport and have concerns or requests about teams you may send a confidential email to athletemanager@daaa.org

Registration Deadline for Individual Events
Registration deadline for individual events additions is 24 hours prior to the event, provided there is room available in the timeline for each sport.  Events can be added on-site at the DAAA registration room in the Hyatt, as long as it is before the 24 hour final deadline.

Early Registration should be completed before June 1, 2018 to receive the discounted registration fees and ensure sport event availability.
 
Online Registration Instructions:
  1. Complete Medical Evaluation Form prior to arrival. Email signed form to Registration@daaa.org or bring it with you to onsite registration.
  2. Personal Information, T-Shirt Size, and Classification Info (swimming only).
  3. Waiver Acknowledgement.  Code of Conduct will be signed on-site. Athlete must be present at time of signing. 
  4. Pay Registration Fee; determined by age and sport (All sports or just Boccia). Athletes/guardians may pay by using our secure square online portal https://squareup.com/store/DAAA/ .  DAAA will not be processing registration over the phone.
  5. Preferred method of registration is using our online registration.
  6. Email docs as PDF, must include athletes name in the subject line and send to: registration@daaa.org
 
              After June 1st and Onsite
  1. Must completed everything listed above. If there is no completed Medical Evaluation Form brought to on-site registration, the athlete cannot participate.
  2. Pay the late fee in addition to the registration fee.
  3. Understanding that not all sporting events will have availability for additions after June 1st registration.
 Refunds/Cancellations:
Requests for refunds will be possible if communicated to registration@daaa.org email before June 1st, minus a $25 processing fee. Refunds or cancellations after June 1st will not be honored.

    Medical Evaluations

    ***ALL athletes must submit a new Medical Evaluation for 2018 Orlando DAAA Games. Please email your current medical evaluation, or any medical evaluation questions to Registration@daaa.org
    Visit www.daaa.org to download the Medical Evaluation. Completed Document must be submitted to DAAA by May 31st or brought to on-site registration; or athletes will NOT be allowed to participate. 

    Eligibility
    You are eligible for competition if you are a disproportionate dwarf (diagnosed with Achondroplasia) that is less than or equal to five feet in height, or a proportionate dwarf less than or equal to 4 foot 10 inches in height.
    • All Athletes must report to the DAAA Registration Room prior to your first event for National Games Check-in. (Soccer Registration must be completed on Friday evening or 6 A.M. Saturday morning)
    • All Athletes will be assigned a Participation Number that must be presented at check-in for all events.

    Event Selections

    In this section you will select the events in which you want to participate.
    Event Selection
    • Select the events in which you want to participate for each Sport.
    • Scheduled event times are check-in times, athletes must be at the venue by that time.
    • Select from the list of sports below. Be sure to check the calendar of events to avoid any conflicts  
    If you know the name of your team, or your Team Captain, please provide it below. 

    Swimming Events - Sunday July 8th, 2018

    Maximum of 4 individual events -  
    • Select your division and events.  Enter a maximum of four swimming events not counting relays.
    • An athlete will only be able to compete in one division per each event. An athlete may compete in two different divisions for different events.
    • Males and females (all events & different classifications will be combined during the races; however, separate awards will be presented for each gender, class and age group). The Junior and Future kick board and life jacket races are intended only for beginning swimmers who cannot swim 25 meters unassisted. There will be no gender or age in these divisions in these races. An experienced swimmer may assist the racer. Athletes are required to bring their own flotation devices.

    Volunteers: We need volunteers. If you would like to volunteer please complete and submit the volunteer form here

    Transportation for Athletes: DAAA has contracted a charted shuttle for the three days of off-location sports: Soccer, Badminton/Table Tennis, and Basketball. Please note that swimming is within walking distance. Transportation is not provided for swimming. Policy for transportation: athletes are prioritized (show athlete number) and given free transportation on the charted shuttle bus.
    Transportation for Parents and Spectators must pay: One parent of a child under 12 years old is given a pass for free, spectators and additional family members must purchase a bus pass.  Children under 5 years old are waived, with a paid adult.
    • Proof of bus pass must be shown in order to ride DAAA’s shuttle bus. Bus passes will be enforced.
    • Daily bus passes will be sold at the bus stop at the rate of $10 per person. Purchase early for best rate.
